About these terms

This Website is intended for healthcare professionals and individuals involved in patient access and reimbursement support in Canada, including physicians, nurses, pharmacists, pharmacy technicians, social workers, drug access navigators, and other healthcare stakeholders, in each case acting in a professional capacity. The Website is not intended for patients or members of the public. Patients with questions about their medication, treatment or coverage should speak with their own healthcare provider or care team.

Registration and accounts

Registered areas. Certain areas of the Website, including the Enrollment Form Hub and the training modules, are available only to registered users. You must not access, or attempt to access, any registered area unless you hold an active account in your own name.

Eligibility. Registration is open only to healthcare professionals, and to employees of healthcare organizations and public sector bodies, in each case in Canada and acting in a professional capacity. We decide in our sole discretion whether to approve a registration, and we may decline any registration without giving reasons.

Registration information. You must register in your own name, using your own work email address. All information you provide must be true, accurate, current and complete, and you must keep it up to date through your user profile. You must not register on behalf of another person or hold more than one account.

Verification. We may, but are not obliged to, verify the information you provide, including your professional role and workplace, and we may ask you for further information for that purpose. If you do not provide it, we may decline, suspend or close your account.

Account security. Your username, password and any other security information are confidential and personal to you. You must not disclose them to, or allow your account to be used by, any other person. You must take care when accessing your account from a public or shared device, log out at the end of each session, and notify us immediately at info@sentrex.com if you become aware of any unauthorized access to or use of your account or any other breach of security. You are responsible for all activity that occurs under your account.

Training records. Training content is provided for professional education only. Unless we expressly state otherwise for a particular module, completing a module is not an accredited continuing education or continuing professional development credit, and it does not confer any professional designation, licence or qualification.

Suspension and termination. We may decline, suspend, disable or terminate any account, username or password, or your access to all or any part of the Website, at any time and in our sole discretion, with or without notice, including if we believe you are not eligible, have provided inaccurate information, have shared your account or have breached these Terms of Use. You may close your account at any time by contacting us at info@sentrex.com. When your account is suspended or closed, your right to access the registered areas of the Website ends immediately. We will handle the personal information associated with your account in accordance with our Privacy Policy.

Use in Canada

The Website is operated by Sentrex Health Solutions Inc. from Ontario, Canada, and is intended for use only by persons located in Canada. We make no representation that the Website or its content is appropriate or available for use outside Canada. If you access the Website from outside Canada, you do so at your own risk and you are responsible for complying with the laws of your jurisdiction.

Information and professional education only

The content on this Website is provided for general information and professional education only. It is not medical, clinical, pharmaceutical or coverage advice, it is not a substitute for the professional judgment of a qualified healthcare provider, and it does not guarantee coverage, eligibility, funding or any particular outcome for any patient.

Healthcare professionals should exercise their own professional judgment and confirm all patient-specific information through the applicable manufacturer, payer, patient support program, or other appropriate authority.

Coverage and reimbursement

Information on this Website does not guarantee patient eligibility, reimbursement, funding, coverage approval, enrollment in a patient support program, or any specific outcome. Coverage and funding decisions are determined solely by the applicable payer, manufacturer, program sponsor, or governmental authority.

Medical emergencies

This Website and its contact form are not monitored for medical emergencies.

If you are experiencing a medical emergency, call 911 or go to the nearest emergency department.

For urgent medical advice that is not an emergency, contact an appropriate healthcare provider or service available in your area.

Do not submit personal health information about any patient through the Website, including through the contact form.

Accuracy and currency of information

We make reasonable efforts to provide accurate and up-to-date information. However, enrollment forms, formularies, program criteria and funding rules change frequently. Information on this Website may not be current. You must confirm the current form, criteria and process directly with the applicable manufacturer, patient support program or payer before relying on anything published here.

Enrollment forms and patient support programs

Enrollment forms are published on the Website as a convenience for healthcare professionals. Each form is the property of the manufacturer that issues it. Each Patient Support Program is offered by the manufacturer that sponsors it and is operated by or on behalf of that manufacturer, under that manufacturer's own terms and eligibility criteria. Sentrex Health Solutions Inc. operates some of these programs, including Haven by Sentrex. Publishing a form here is not an endorsement of the product or the program, and it does not mean we operate it. Forms change without notice. Confirm you are using the manufacturer's current form, and confirm program eligibility and criteria with the manufacturer, before you submit anything for a patient. Nothing in these Terms of Use or on the Website is medical, clinical, pharmaceutical or coverage advice, and nothing on the Website guarantees coverage, eligibility or funding for any patient.

Third-party products, programs and organizations

All product, program and organization names, logos and brands referenced on this Website are the property of their respective owners. Reference to a third-party product, program or organization does not imply any affiliation with Sentrex Health Solutions Inc., and it does not imply that we endorse that third party or its products, and does not imply that a third party endorses any of Sentrex's products or services.

Intellectual property

Unless otherwise indicated, the Website and its original content are owned by Sentrex Health Solutions Inc. or its licensors. This includes:

You may not reproduce, republish, distribute, modify, sell, license or commercially exploit Website content without prior written permission from Sentrex Health Solutions Inc. or the applicable rights holder.

Enrollment forms published on the Website remain the property of the manufacturers that issue them. You may download and complete an enrollment form only to enroll a patient in the applicable program, and you must use it in accordance with that manufacturer's terms.

If you copy, download or use any part of the Website in breach of these Terms of Use, your right to use the Website ends immediately and you must, at our option, return or destroy any copies you have made. All rights not expressly granted in these Terms of Use are reserved.

No permission is granted to copy, download, reproduce or reuse licensed photography except as part of viewing the Website in the ordinary manner.

Trademarks

Drug Access Canada, Haven by Sentrex and PerCuro by Sentrex are trade names and trademarks of Sentrex Health Solutions Inc. You must not use them without our prior written permission.

All other trademarks, service marks, logos, and brand names remain the property of their respective owners.

This Website may contain links to external websites and third-party resources. External websites are governed by their own terms, privacy policies and accessibility practices. We do not control and is not responsible for:

Except for personal information, which we handle in accordance with our Privacy Policy, Submissions are not confidential. You grant Sentrex Health Solutions Inc. and its affiliates and service providers a worldwide, royalty-free, perpetual, irrevocable, non-exclusive licence to use, reproduce, modify, display, publish and distribute your Submission in connection with the Website and our business. To the extent permitted by law, you waive any moral rights in your Submission.

We are not obliged to publish, use or respond to any Submission, and we may refuse, edit or remove any Submission at any time in our sole discretion.

We may modify, suspend or discontinue any part of the Website at any time. We are not liable if all or any part of the Website is unavailable at any time or for any period.

We may also update these Terms of Use. Revised terms take effect when posted on this page with an updated “Last updated” date. Your continued use of the Website after revised terms are posted constitutes acceptance of the updated terms.

Governing law

These Terms of Use and your use of the Website are governed by the laws of the Province of Ontario and the federal laws of Canada applicable in Ontario, without regard to conflict of laws principles.

Any action or proceeding arising out of or relating to the Website or these Terms of Use must be brought exclusively in the courts of the Province of Ontario, and you irrevocably submit to the exclusive jurisdiction of those courts.

Severability

If any provision is found invalid or unenforceable, the remaining provisions will remain in full force and effect.

Waiver. No failure or delay by us in exercising any right or remedy under these Terms of Use operates as a waiver of it, and no single or partial exercise of any right or remedy prevents any further exercise of it or of any other right or remedy.

Entire agreement. These Terms of Use, our Privacy Policy and any additional terms that apply to particular areas or features of the Website, including our Manufacturer Submission Terms, are the entire agreement between you and Sentrex Health Solutions Inc. regarding the Website and supersede all prior understandings regarding it.

Survival. Any provision of these Terms of Use that by its nature should survive the suspension or closure of your account or the end of your use of the Website survives, including the sections on Submissions, Intellectual property, Disclaimer and limitation of liability, Indemnification and Governing law.
